About the Role

Vancouver Coastal Health (VCH) is committed to supporting a strong foundation of primary and community care. Since launching our first Urgent and Primary Care Centre (UPCC) in 2018, VCH now has three centres across our urban and metro areas aligned with the Ministry of Health UPCC policy direction.

We expect UPCCs to take pressure off Emergency Departments for lower-acuity cases as well as provide
key linkages for patients to primary care. The following principles guides the VCH Urgent and Primary Care Centre (UPCC) service delivery model and Clinical Service Plan.

KEY PRINCIPLES
1. UPCCs will provide accessible, high quality, culturally safe, patient-centered care.
2. UPCCs will be VCH operated with exception of existing contracted sites.
3. The service that each UPCC offers, the hours of operation, and the general staffing model is standardized across VCH to ensure consistency for our public receiving care anywhere in VCH.
4. The core services at each site will include Urgent Primary Care, Diagnostics and Comprehensive
Longitudinal Primary Care (CLPC).
5. The UPCCs will enable patient attachment in collaboration with Division of Family Practice.
6. Each UPCC is part of a PCN and is integrated within the continuum of VCH care services.
7. The UPCC services will match services with community needs and optimize team-based care to increase capacity.
8. UPCCs will create an environment which embraces team based care, teaching, cultural safety, diversity, equity and inclusion for patients and for staff, which is paramount to providing an equitable and quality health, experience and quality outcomes.
9. UPCCs will continually improve patient care, informed by continual measurement of quality metrics and data-driven evaluation dashboards.
10. A UPCC project team will include embedded permanent resources that will transition to sustainment resources after the project is completed.

Requirements

  • Responsible for project management over local technical readiness for UPCC as requested by Project

    Manager and CST Director

  • Health organization engagement, planning, oversight and management relative to technical readiness.

  • Specifically oversight and management to:

    • Device assessment

    • Device proposal

    • Device procurement

    • Device deployment

    • Device activation

  • Device transition to sustainment model

  • Alignment with enterprise device team processes and standards

  • Alignment/liaising with IMITS for network/infrastructure remediation efforts

  • Alignment/liaising with Biomed for integration readiness efforts


  • Alignment/liaising with other technical teams as required

  • Device financial forecasting in partnership/as requested by health organization
